Our Patient Portal

Plano Primary Care enhances your healthcare experience by providing secure and easy access to your medical records online. Our Patient Portal gives you access to your health information anytime, anywhere, with an internet connection. Stay informed with periodic updates and reminders directly from your doctor, all conveniently delivered to your email.

To gain access to our Patient Portal, kindly contact our medical clinic to set up your account using a permanent email address you check regularly. You’ll receive a unique username and password, allowing seamless access.

 
For password changes, simply use the “change/reset password” feature on our Home page. However, access will be temporarily restricted until our staff approves the update.
For security reasons, three incorrect password attempts will lock your Patient Portal access. Contact our medical clinic at (972) 599-0400 to unlock your account.
  • The Patient Portal is for non-emergency use only. 
  • The Patient Portal is only checked during normal business hours. It is not checked on weekends and holidays. 
  • Please allow us up to 72 hours to respond to your requests. 
  • While the Patient Portal facilitates communication, it does not replace your regularly scheduled doctor’s appointments. 
  • We do not send any private information to your email address. 
  • When private healthcare information has been posted on your Patient Portal, you will receive an email requesting you to review the data.
